Are you passionate about second chances? We are seeking two dedicated Case Managers to work our prisoner re- entry grant, connecting inmates and recently released offenders with resources and services to help them rebuild their lives after incarceration. At Catholic Charities, we believe in the diginity of each person and structure our prisoner re-entry program to help former inmates achieve their full potential as human persons. As a Case Manager working in prisoner re-entry, you'll have the opportunity to make a meaningful impact in the lives of former inmates. 

You will be responsible for:

• Enrolling approved candidates in the prisoner re-entry program and developing individualized re-entry plans. 
• Helping former inmates access a variety of services including: assistance with rent, rent deposits, and utility bills; workforce documents such as State IDs, birth certificates, TWIC and Safety Cards; resume writing and employment assistance; transportation needs; and enrollment in driver's training and educational/workforce training programs.
• Maintaining case files, tracking and reporting data, in accord with agency and grant policies.
• Participation in stakeholder meetings, job fairs, home and prison visits, and other community engagement activities. 

Qualifications needed:

• Bachelor's degree or higher in Social Work or Criminal Justice, or related field (preferred)
• Strong communication and interpersonal skills
• Ability to make strong prudential judgements and maintain professional boundaries.
• Demonstrated experience (2-3 years) in community building and experience working with grants.
• Ability to work weekdays 8:30AM to 4:30Pm with flexibility for occasional evenings to meet client needs.
